Legacy Fund Grant Application Form

  

       Information:

1.  Only non-profit, 501(c)3 organizations will be considered for grants.
2.  Attach a copy of your 501(c)3 determination letter to the application.
3.  Grants for "brick and mortar" will rarely be awarded. 
4.  Grants in excess of $3000 will rarely be awarded. 
5.  Application for a grant must be made using the Bethel UCC Legacy Fund Grant Application Form that may be obtained from:

e-mail or from our  website,

www.betheluccwat.org.  However, a paper copy with original    signatures must be sent to the above address for the application to be considered.

 

1. If not associated with Bethel UCC, attach a complete description of your organization to the application.    Include the names of officers and directors and any other information that you think would be helpful. 
2. Attach complete details of the project to the application.  Include estimated budget, short and long term objectives and proposed schedule with milestones and completion date. 
3. If an organization has multiple projects, a separate application must be filed for each project.  
4. The application deadline for 2009 calendar year is August 31, 2008. 
5. If awarded a grant, you will be notified by December 31, 2008. 
6. If awarded a grant, your organization must return the Request for Grant Release form (to be supplied) to the above address for the grant check to be released.  The deadline for returning the form is June 30, 2008. 
7. If your organization receives grant funds, you must file a Recipient Report form (to be supplied) at the above address prior to November 1, 2008, to be eligible for a grant the following year. 
8. A new application must be submitted each year to be considered for funds.
9. Are your project objectives clearly stated? 
10. Are your objectives measurable? 
11. Can your project be completed within a one-year time frame? 
12. Does your application have the organization's name, address and phone number? 
13. Have you listed a contact person with address and phone number?
